Online Consent To Receive Form 1095-C for Calendar Year 2024

In accordance with the Affordable Care Act (ACA) provisions, UT Dallas will have the 2024 Form 1095-C available by Jan. 31, 2025. Form 1095-C (pdf)  includes information about the health insurance coverage offered to employees, retirees and their dependents. You will receive Form 1095-C if you were enrolled in the UT SELECT Medical Plan or if you worked an average of 30 or more hours per week or were considered a full-time employee in 2024

Retirees enrolled in the UT CARE Medicare PPO Plan will not receive Form 1095-C from UT Dallas or Blue Cross and Blue Shield of Texas. Medicare will send Form 1095 to retirees enrolled in UT CARE Medicare PPO Plan.  

Graduate student employees who are covered under the UT Student Health Insurance Plan can download Form 1095-B from the Blue Access for Members (BAM) website. Members also can call the BCBSTX number on the back of their member ID card to request a physical copy or visit the 1095-B information page for more options.

To provide online consent, go to My UT Benefits and click on “Log in with institution credentials (SSO).” Select “Quick Links” from the My UT Benefits welcome page and select “Your Current 1095-C Form.” Select “Manage Electronic Tax Statement Options” and follow the instructions. If you provided consent in 2023, you do not have to provide consent again. 

If you provide your online consent by Jan. 8, 2025, or previously provided online consent in 2023, you can access your Form 1095-C through My UT Benefits using the “Log in with institution credentials (SSO).” Select “Quick Links” from the My UT Benefits welcome page, then select “Your Current 1095-C Form” and choose “Access Current Year-End Statements.”   

The deadline to provide online consent is Jan. 8, 2025. If you do not provide online consent, you will receive the form in the mail by the IRS deadline. The forms are expected to be available online by Jan. 31, 2025.
